What is Young Voices?
Young Voices is a youth group in West Sussex made up of young people with additional needs (including SEND and neurodivergent conditions) up to the age of 25. Our aim is to influence positive change within WSCC services whilst also having fun and making friends.
We meet every fortnight on Thursdays at 5pm-6.30pm and our meetings are hybrid; in person and online.

What is Young Voices?

You do not have to have a diagnosis or an EHCP to join us as we follow the social model of disability.

Overview
This form is for young people who would like to join the Young Voices Youth forum and/or would like to take part in the SEND ( special educational needs and disabilities ) and Inclusion Partnership Board as youth representatives.
Important note. You will be asked for a phone number for an emergency contact and also brief details of any medical conditions you might have. Please have these details with you to help you complete the form. For young people aged under 16, a parent or guardian will also be asked to give their consent for you to take part. The information on this form will be kept securely and is confidential .
You will be asked to renew this form every two years, to make sure we have the correct details and you still want to take part.
We need the information on this form to make sure you are safe, can take part fully, and that you have given us consent to use your comments and photos.
