Overview

This form is for young people and families taking part in Young Voices workshops, events and projects . We need the information on this form to make sure you are safe, can take part fully, and that you have given us consent to use your comments and photos. 

For young people aged under 16, a parent or guardian  will also be asked to give their consent for you to take part.  The information on this form will be kept securely. You can ask for your details to be removed at any time by contacting voice@westsussex.gov.uk
