Overview

We are doing an evaluation of the Independent Visitor Scheme to make sure that you and other stakeholders are getting what you need from the scheme.

We would really appreciate your honest answers to this survey to make sure that we understand the good parts of the scheme and where it could be made even better. The survey results will be reviewed by the managers of the IV scheme.

Data Protection/Privacy Statement

The Independent Visitor Scheme have created this evaluation in order to review our service in accordance with National Standards for the Provision of Independent Visitor Services, 2016.

The evaluation will involve the collection of your personal data which will be processed in accordance with the Data Protection Act 1998 and any subsequent replacement legislation, including the General Data Protection Regulations.

The personal data we collect will not be shared with anyone outside of West Sussex County Council and will be held securely on a computer for a period of up to 1 year before being appropriately destroyed. Personal data will not be retained for longer than necessary in relation to the purposes for which they are collected. West Sussex County Council is registered as Data Controller (Reg. No. Z6413427). Hub Etiquette
