Key dates:  This survey will be available from 16th March to 14th April 2023. 

Thank you for completing this survey.  The answers you give will not have any negative impact on you. They will be collected together with answers from Children, and Independent Visitors (IVs) to give us an overall evaluation of how the scheme is running.

When we write up the results of the surveys, your name won’t appear – you will be anonymous.

Some questions will ask you to give a score based on how good or bad things are. Other questions will ask you to give more detail. There are no right answers to these questions so please just give an honest answer about how you feel.

The survey shouldn't take longer than 15 minutes to complete.

The Independent Visitor Scheme have created this evaluation in order to review our service in accordance with National Standards for the Provision of Independent Visitor Services, 2016.

The evaluation will involve the collection of your personal data which will be processed in accordance with the Data Protection Act 1998 and any subsequent replacement legislation, including the General Data Protection Regulations.

The personal data we collect will not be shared with anyone outside of West Sussex County Council and will be held securely on a computer for a period of up to 1 year before being appropriately destroyed. Personal data will not be retained for longer than necessary in relation to the purposes for which they are collected. West Sussex County Council is registered as Data Controller (Reg. No. Z6413427).
