Nominations received between Friday 5 January 2024 and Friday 30 August 2024 will be entered for the 2024 Care Leaver Celebration Event (which will take place in October 2024).

Any received after this time will be entered into 2025's event. Please note this is because of the vast amount of organising we have to do, so please ensure you allow enough time for your submissions.

This online form is for adults to nominate young people. You can nominate up to 5 young people on this one form. If you wish to submit more than 5 at once you will have to submit another form.

Are you a care leaver aged 16-25? The new care leaver advisory board would really like to know what you think about services that effect you. With your feedback, we as a group can work on campaigning to change what you want and need. We can hold services to account and together make a difference to you and many other care leavers from West Sussex. 

Please give us your opinions to be in a chance to win a £50 voucher - don’t forget to leave your name to be able to be entered into the draw. If you don't want to be entered into the draw you can fill the form out anonymously.

Congratulations on your nomination for an award at the Care Leavers Celebration Event. We are really proud of your achievements and look forward to celebrating the evening with you. This will be a chance to celebrate your success and that of other Care Leavers, along with your PA. 

We are able to offer you 1 ticket for yourself if you would like to go.

Young people from the Care Leaver's Advisory Board (CLAB) have chosen the colour scheme which is... RED AND SILVER, so please feel free to wear your best outfits, or whatever you feel most comfortable in. 

The evening will start from 5pm where you will have time to mingle with your PA and other Care Leavers, so please feel free to start arriving from this time. At 6pm, you will be served an incredible 3 course meal. After this, we will hold a short ceremony where we will be giving out trophies and certificates and announcing the winners of each category. From around 8pm there will be a disco until 10pm which is when the event will finish.

The event will be held at Avisford Park Hotel, Yapton Ln, Walberton, Arundel BN18 0LS. Please report to reception and we will come and show you the way. There is ample parking on site, or if you are planning on getting the train or another mode of transport, please let your PA know as we will be offering a shuttle bus system on the night to and from Barnham Station.
