With the community fund programme winding down, we need to gain a picture of how successful the programme has been in reaching its aims of meaningful activities, engagement, co-production and empowering those involved.
This survey will ask several questions about your views, and has been co-developed with members of the CAPITAL project team. It should take no longer than fifteen minutes. Results will help us to understand if improvements need to be made in future, and what the strengths or weaknesses of the model are.
You do not need to give us your name and responses are anonymous. The responses are only viewed by the co-production management team from CAPITAL and West Sussex County Council's Public Health Team. If you mention anything in your feedback that might make it possible to identify yourself or others, we will make sure that this is removed before any reports are published.
Your responses, positive or negative, will have no impact on your involvement moving forwards.
