Involving young people and their voice in your work - Request Form

Overview

Making sure young people's voice is a key part of making your services and projects a success, as well as being a legal requirement in some cases. This form has been created for completion by those who would like to involve young people and support them to get their voice heard. The earlier in the process you consider how you would like to involve young people the better. 

There are many ways of doing this, and many things that you will need to consider as a consultor.

Before you complete this request, please read the attached sheet Helpsheet for Consultation with young people listed in the "Important links" section to the right of the page and at https://yourvoice.westsussex.gov.uk/7694/widgets/28844/documents/13364  

We think this form will take less than five minutes for you to complete if you have all the information we need. The Voice and Participation team receive a large number of requests for young people's support. Please allow 14 days to receive a response to your request.  In case of query please contact voice@westsussex.gov.uk

The Voice and Participation team will use this questionnaire to collect personal and special category data the processing of which is necessary to enable us to be compliant with the law (general well-being duties), and for reasons of substantial public interest.  The data will be processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations, and any subsequent data protection legislation. The data we collect be securely stored and kept for period of up to 2 years before being appropriately destroyed.  The data we will collect will not be used for any other purpose than that stated above without your prior written consent.
