This form is for adults taking part in sessions and work run by the Voice and Participation Team within Early Help of the West Sussex County Council. The Voice Lead, who is based in the Voice and Participation Team needs the information on this form to make sure you are safe, can take part fully, and that you have given us consent to use your comments, photos and videos.
