
Voice and Participation Team Request Form
This form is to be used if you would like to:
1. Request to borrow a specific piece or pieces of equipment from the Voice and Participation Team (V&P Team)
2. Request training from the V&P Team
3. Request a delivery of positive postcards or comfort cases
4. Request something to be posted on our V&P social media channels.
PLEASE NOTE FOR Equipment Borrowing
- It is really important to us to track and monitor the use of what we have so that we can ensure it comes back working and fully charged ready for the next team.
- You can only use the equipment for events/projects that support young people having their voice heard or promotes their engagement in West Sussex County Council services. If you need to use it for something else, just let us know first and we will consider if this is appropriate.
- Items loaned must be returned in the condition they were supplied. In the event that something is damaged or broken the hiring service area will be charged for the total cost of repair or replacement
