As part of our evaluation of the Taskforce Pilot Project, we want to understand the impact we have had in your school and with your young people. Please be as honest as possible in your answers. Gathering this information will help us to understand our progress, and hopefully help us to continue with the Taskforce.

We will collect some personal data in fulfilment of the exercise of official authority vested in the controller. Data collected will be processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations, and any subsequent data protection legislation.

The data we collect will not be used for any purpose other than that stated above without your prior written consent. The data we collect will be securely stored by us for a period of up to two years before being appropriately destroyed.
