As part of the review of SEND being undertaken by a West Sussex Local Authority SEND & Inclusion Adviser at your school, we would like to gather staff views.

The responses are anonymous and will be used to support the school’s development of their SEND provision and practice. In accordance with the General Data Protection Regulations (GDPR), this data will not be shared or used for any other purpose than what has been detailed above. Once collated the individual responses will be deleted. 

The SEND and Inclusion Service will collect some personal data in fulfilment of the statutory duty for the performance of tasks carried out in the public interest or in the exercise of official authority vested in the controller. Data collected will be processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations, and any subsequent data protection legislation. 

The data we collect will not be used for any purpose other than that stated above without your prior written consent. The data we collect will be securely stored by us for a period of up to two years before being appropriately destroyed.
