Individual Support Plan Engagement 2024 to 2025
Thank you for taking the time to complete this survey. If you are using or considering using the Individual Support Plan or are considering how to work more effectively within the graduated response to supporting children with SEND within your setting, completion of this survey will provide you with access to additional support, advice and resources over the coming academic year.
The aim of this survey is to:
- Have oversight of which schools in West Sussex are using or thinking of using the Individual Support Plan within their settings to inform future planning.
- To have the contact details and email addresses of settings who would like updates on new resources to support their implementation of the graduated response, so that we can share resources and peer support events.
- To identify any areas for development in terms of resources or training in the coming year which would support your practice.
The Individual Support Plan team includes representation from the following teams and services:
Autism and Social Communication Team
Early Years SEND Transition Team
Educational Psychology Service
Learning Behaviour Advisory Team
SEND and Inclusion Team
In addition to raising awareness of new resources, the team will be offering peer support events to provide opportunities for the sharing of good practice and problem solving.
The Survey should take no more than 5 minutes to complete and the contact details shared will be used to add your names and addresses to our group email so that we can update you on new developments.
