Provider Forum - NHS Health Checks and Smoking Cessation

Location: The Charis Centre, Crawley (RH11 7EL) - parking on site

This aims to be a unique annual training day to support and educate providers on the existing smoking cessation and NHS Health Check services you are delivering. Lunch and refreshments will be provided.

Please complete the registration form at the bottom of this page by Friday 23rd August. Agenda to follow. The day will also contribute to your on-going CPD learning to ensure maximum benefit of attendance.

Please feel free to share this invitation with other Health Checks and Smoking Cessation service providers in West Sussex. However, please note that we can only accept two people per organisation in the first instance, however others may be added to a waiting list and offered a place later on, depending on demand.
