Your views about Hospital Discharge (Home First) Care Services

We have contacted you because you are a key stakeholder for the West Sussex Community Hospital Discharge (Home First) care services . We want to improve our services and make plans for the future and we therefore need to find out about your experiences and what effect Community Reablement Services have had on the lives of your customers/patients.

The surveys are being conducted by West Sussex County Council. It should take around 10 minutes to answer the questions.

We would like to hear from as many people as possible, but the surveys are voluntary. You do not have to answer the questions if you would prefer not to. It will not affect the services you receive if you do not participate.

The surveys close on Thursday 2 May 2024.

What we will do with the results of the surveys

What you tell us will help us to understand how well the Hospital Discharge (Home First) Care Services are performing and whether any improvements are needed for the future. The results will be used to inform the award of a new contract for delivering the services in 2025.
