Overview

What is Changing Futures?

Changing Futures is a Sussex-wide system change programme focussing on improving systems and services for people experiencing multiple disadvantage, which means adults experiencing three or more of the following five: homelessness, substance misuse, mental health issues, domestic abuse, and contact with the criminal justice system. It is funded by DLUHC (Department for Levelling Up, Housing and Communities) until March 2024.

What will this journey map be used for?

We are recording and mapping people’s journeys and experiences through, and interactions with, services to understand what works well and what doesn’t work. To understand where systems and services need to improve, be responsive and more timely for people with multiple disadvantages. We want to find patterns showing blockages or successes, so that they can be worked through or learnings passed to other services.

Who should be filling it in? Is it done on one’s own or with someone else?

We are offering a variety of options; it can be filled in by individuals themselves, it can also be completed by a support worker in conversation with the client, we can also make arrangements to facilitate a focus group and it can be filled in on line through this page. A downloadable Word copy is available on through the link to the right of this page as well.

What will happen with this information?

Journeys are anonymous; we are asking for some details such as gender, age and location, so that we can check if there are any patterns based on these. With all the information collated, we will create some examples of common journeys, these will be used to build our service and system change work. It will also be shared across relevant agencies that can support system change.

Is it an overview or should it be more in depth?

Any input is valuable, but details help us drill into problems and successes with more insight. If you think it’s relevant or useful – please include it!

What are we hoping to achieve?

We are hoping to find successes and sticking points in systems, and find ways that organisations can work better together to eventually offer people a simpler, more navigable journey through services.
