Your views matter - Mental Health Act assessment

We want to hear your views. We invite you to take part in our survey of adults who have had a Mental Health Act assessment in West Sussex. The survey is being carried out by Adults' Services in West Sussex County Council. 

The questionnaire has been sent to you because you are an adult and you have had a Mental Health Act assessment during the financial year form April 2025 to the end of March 2026.

The questionnaire asks about your views on the Mental Health Act assessment and how the Approved Mental Health Professional [AMHP] supported you in that assessment.

You do not have to answer all of the questions; if you don't want to answer a question, or don't know the answer, then move on to the next question. It should take you no longer than 10 minutes to complete the questionnaire. When it is complete send it back to us in the pre-paid envelope. You do not need to put a stamp on the envelope.
