Young Carers Transport Agreement

This form is for the parent/guardian of young people requiring transport provided by volunteers to access weekly term time groups run by the Young Carers Team within Children, Young People and Learning of West Sussex County Council. The Lead, who is based in the Young Carers Team needs the information on this form to make sure your child and the volunteer are safe and that you have understood what is expected of you and your child in using this service.
