My Future Activities Agreement Form 18+

Overview

This form is for young adult carers (18-20 and 21-24 year olds) taking part in My Future activity sessions both in person and/or online using video conferencing software and work run by the WSCC Young Carers Team in partnership with Carers Support West Sussex. The Lead, who is based in the Young Carers Team needs the information on this form to make sure you (18-20 and 21-24 year olds)  are safe, can take part fully, and that you have given us agreement to use the appropriate online system.
