Satisfaction Survey

This survey will collect some personal data (e.g. the name of your Smoking Advisor), the processing of which is in fulfilment of a task carried out in the public interest. We will process this data in accordance with the 2018 Data Protection Act, General Data Protection Regulations and any subsequent relevant legislation. The Public Health Team has a specific Public Health Privacy Policy (opens in new window) which explains how personal information is going to be used, what it is used for, who it might be shared with and why, and for how long it is kept.
