West Sussex County Council is working closely with the NHS on the COVID-19 vaccination programme for all health and social care worker in West Sussex. Please note however, that the NHS are responsible for delivering the vaccination programme.
We have identified that your organisation employs social care workers who work in West Sussex and are eligible to be vaccinated under Priority Group 2 (frontline health and social care workers) as set out by the Joint Committee on Vaccination and Immunisation (JCVI).
Please urgently complete this survey so we can support your organisation to access the Sussex NHS COVID-19 vaccination programme. By completing and returning this survey you consent to us providing the details to the NHS to support COVID-19 Vaccination. To make sure all eligible social care workers working in West Sussex have had the opportunity to be vaccinated we will require you to continue to complete this short survey on a regular basis. This will ensure all eligible social care providers have been able to access the NHS vaccination programme.

To enable WSCC to support the rollout of the Covid19 vaccine across the frontline social care workforce, we want to gather information on who has had a vaccination.
