Thought-Full PERMA Workplace Wellbeing Questionnaire 2025

Seligman (2011) suggested that there are five building blocks that enable flourishing – Positive Emotion, Engagement, Relationships, Meaning, and Accomplishment (hence PERMA™). As a service we would like to understand how the team as a whole feels about their work and make any improvements we can.
After completing the individual questionnaire we would like you to record your average percentage score for each section (numbers in column H in the spreadsheet) against the questions below.
Your responses will be anonymous but we would like you to indicate which area team you work in so that we can look at responses at an area team level as well as a service level. Please use the information from this questionnaire to reflect on your workplace well being objective in your appraisal document and consider next years objectives.
