Introduction

Satisfaction surveys are one of a variety of approaches to receiving feedback and assuring quality. Other methods include service reviews/audits/inspections and person-centred review meetings. By using a range of approaches, we can ensure that the widest possible perspective of how services are received can be obtained.

The ‘Tell us what you think survey’ is an annual survey which aims to give an overall picture of our Directly Provided Services of the areas which are working, and where we can improve.

We welcome your thoughts/comments regarding the quality of the service and would be very grateful if you could find a few minutes to fill in this form.

The Adults Directly Provided Services team will use this survey to collect some personal data (eg. name of your service, age etc) which is necessary in the exercise of official authority vested in the Controller. The information you provide will be used for the purposes stated above and treated in the strictest confidence. The information will be processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations, and any subsequent data protection legislation.

The data we collect will be securely stored and kept for period of up to 2 years before being appropriately destroyed. The data we will collect will not be used for any other purpose than that stated above without your prior written consent. West Sussex County Council is registered as Data Controller (Reg.
