Data Protection/Privacy Notice
The Adults Directly Provided Services team will use this survey to collect some personal data (eg. name of your service, age etc) which is necessary in the exercise of official authority vested in the Controller. The information you provide will be used for the purposes stated above and treated in the strictest confidence. The information will be processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations, and any subsequent data protection legislation.
The information you provided will be stored securely by the Council for 2 years before being appropriately destroyed. Your details will not be used for any other purpose without your prior written permission.
