Please scroll down to 'FALLS QUESTIONNAIRE' below to submit your thoughts on areas for quality improvement supporting falls prevention, response, treatment and care and any enablers or barriers for these. The deadline has been extended to Friday 7th November. Responses will be collated and reported at the Sussex Falls Workshop on Wednesday 19th November. The Public Health Team will use the survey questionnaire to collect some personal data (your name, work email address and role title) the processing of which is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller. The data collected will be processed in accordance with the Data Protection Act 2018, the General Data Protection Regulations (UK) and any relevant subsequent legislation. We will ensure your data is processed fairly and lawfully, is accurate, is kept secure and is retained for no longer than is necessary. West Sussex County Council is registered as a “Data Controller” with the Information Commissioner's Office (Reg. No. Z6413427).
