Strengths and Difficulties Questionnaires
Please see below a link to an online questionnaire. The completion of the questionnaire is an annual requirement from Central Government to collect information on the mental health of children aged 4 -17 (aged 16 as of 1st of April and children aged 4 years as of 1st of April) who have been in care for one year or more.

We are required to ask all carers and children aged 11 to 16 to complete a standardised screening tool.
The SDQ asks questions about emotional health, conduct, hyperactivity, peer relationships and social behaviours. The answers render a total score which can be used to inform us about the strengths and difficulties of a child in these 5 domains.
In order to gain a fuller picture your Social Worker will follow up on scores higher than the normal range, this will be in liaison with the NHS Looked After Nurse team and may lead to a referral to the Therapeutic Access Panel, more support in placement or school, or another strategy as appropriate.
