Provider Safeguarding Questionnaire (September 2026 onwards)

West Sussex Safeguarding Adults Board will use this survey to collect some personal data (e.g. name, email address)  which is necessary for the purposes of legitimate interests pursued by the controller or a third party, except where such interests are overridden by the interests, rights or freedoms of the data subject.   

All personal data will be processed in accordance with processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations and any relevant subsequent legislation.  It will not be used for any other purpose without your prior written consent.  The information we do collect will be stored electronically for a period of up to 5 years before being appropriately destroyed. The Safeguarding Adults Board do not respond directly to safeguarding or quality concerns or whistleblowing.
