Oral health survey - children 5 and under
Overview
Thank you for your interest in completing this survey.
Good oral health is important for all our population, especially children. We know that creating good habits around toothbrushing and visiting the dentist at an early age will support a child's oral health.
West Sussex County Councils' Public health team want to find out more about oral health practices and experiences of parents and caregivers who have children 5 and under. This information will be used to inform the support and services we provide for children’s oral health in West Sussex.
We invite you to take part in this short anonymous survey to support this work within West Sussex. Hearing directly from people in our local population is extremely valuable. Your responses will not be able to identify you, and you will not be contacted regarding the answers you provide.
This survey is open to be completed by parents and carers aged 16 and above of children 5 and under who live or go to school in West Sussex. You may complete and submit one survey for each child you have that is 5 and under, however you are permitted to place only one entry into the prize draw.
We want to understand more about children's toothbrushing routines and dentist visits, and any challenges you face with implementing either of those activities.

To thank you for your time in completing the survey you will be eligible to enter a prize draw to win one of five £20 One4all gift vouchers as a thank you! Personal information requested to enter will only be seen by the member of staff responsible for distributing the prize and will not be linked with your responses in any way. The survey is anonymous. We will not use your responses to identify you or attempt to contact you in any way. Your responses will not affect any service provision you currently receive.
The survey will close on Friday 12th of June 2026 at 11.55pm.

How we will use the information (data) you give us
We will process this data in accordance with the 2018 Data Protection Act, General Data Protection Regulations and any subsequent relevant legislation. West Sussex County Council is registered as a Data Controller (Reg. No. Z6413427). The data you provide will not be used for any purpose other than that stated and will be stored electronically for a period of up to 1 year before being appropriately destroyed.
