West Sussex County Council collects information about children and young people age 0 - 25 with Special Educational Needs and Disabilities (SEND) to help plan and deliver their services. The Disability Register is maintained by West Sussex Library Service.
Children and young people who have a disability can sign up to the register and can opt to receive a Max Card, offering access to discounts on local and national attractions.
Some of these cards are soon to expire. We are planning to purchase more but answering this short survey would give us an initial idea of demand. Thank you for taking the time to complete it.
This survey should take less than 5 minutes to complete.
