Data Privacy Notice

The Adults Service and Health Team will use this questionnaire to collect personal and special category data, the processing of which is necessary to enable us to comply with our duty to consult. The data will be processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations, and any subsequent data protection legislation.

The data we collect will be securely stored by electronic means and will kept for period of up to 1 year before being appropriately destroyed. The data will not be shared with anyone else without your prior written permission.
