If you are currently experiencing homeless and aged 16 years and older, this survey is for you. Experiences of homelessness may include, living in temporary accommodation, hostels or support accommodation, or other homeless situations, such as sleeping rough or sofa surfing.


We want to hear about your experiences of being homeless and, as a thank-you for completing the survey, you can enter our prize draw to be in with a chance to win one of ten £25 shopping vouchers (Amazon, Love2Shop, or a supermarket voucher). Prize draw entry, and our terms and conditions can be found at the end of the survey.

Taking part in this survey is voluntary. Your answers will be anonymous and not traceable to you. Your answers will not affect any support or services that you are receiving.

The survey will take approximately 20-25 minutes to complete.

Why are we asking for you to share your experiences?

Where we live plays an important role in supporting our overall health and wellbeing. Becoming homeless can affect an individual's health and wellbeing. This can include how easy it is to access services, support, and everyday activities.

West Sussex County Council are working with partner organisations across the housing, health, care, and voluntary sectors to carry out a needs assessment for people experiencing homelessness in the county.

We want to understand our residents' experiences of health and wellbeing during a period of homelessness. This will help us improve local services and support for people experiencing homelessness in the county.

Some people may find some questions uncomfortable or upsetting to think about, but we are asking about them to identify how we can better support people. You do not have to answer any questions that you find upsetting. If you would like advice or support for any of the issues raised, you can find links to local support organisations on the side panel of this page.

By taking part, you will be giving consent for your responses to be used to inform the West Sussex Homelessness Health and Wellbeing Needs Assessment. Because all of your answers will be anonymous, it will not be possible to withdraw your data after submission.

How we will use the information (data) you give us

West Sussex County Council’s Public Health team will collect some personal data in this survey to help us understand the characteristics and experiences of people completing it. The data will be processed in accordance with the 2018 Data Protection Act, the General Data Protection Regulations, and any subsequent relevant legislation. 

The data we collect will not be used for any purpose other than that stated above without your prior written consent. The data we collect will be securely stored by us for a period of up to 2 years before being appropriately destroyed. Data collected to administer the prize draw will be deleted within a month of vouchers being shared with winners.
