Home First Review - share your views

I am writing as you as you have recently been discharged from hospital to receive care at home whilst waiting for an assessment of longer-terms needs. This is known as the ‘Home First’ service.

If you are carer to, or representative of, the person who received the Home First service your opinion is also important to us so could you please respond on their behalf regarding the service they received.

The county council and the NHS work together with domiciliary care agencies to provide support in people’s home for the first few days after they leave hospital. This helps them to continue their recovery before their longer-term care needs are considered.

On behalf of the county council, I am carrying out a review of the Home First service to find out what is working well, and what could be improved.

I am doing this as part of my apprenticeship degree and the findings will be included in a research report I will submit to the University of Chichester as part of my course.

To help me do the review, I am contacting people like you who have recently used the service, or care for someone who has, to ask you to complete a short survey about your experiences.

You do not have to take part in the survey if you do not want to, but I would very much value your opinion on your experiences of the service. Your views will help continue to develop the service.

I will not use your name or any information that could identify you or the person you care for as part of the review or in my report. What you tell me will not affect the services you or the person you care for receive.
