Sign up for SEND green paper Zoom session

Overview

This form is to sign up for the SEND green paper online workshop on Weds 29th June 5 'til 7pm on Zoom . 

 The SEND Participation Lead, who is based in the SEND Commissioning Team needs the information on this form to make sure you are safe, can take part fully, and that you have given us consent to use your comments and photos. 

For young people aged under 16, a parent or guardian  will also be asked to give their consent for you to take part.  The information on this form will be kept securely. You can ask for your details to be removed at any time by contacting SENDCommissioningTeam@westsussex.gov.uk
