The Family Information Service has been notified that you intend to stop, or have already stopped, childminding. This information has either come from yourself or Ofsted have informed us. We continually strive to improve the service we offer to our childminders and would be grateful if you could please complete the short questionnaire to help us improve support to childminders in the future.
