Your response to this survey is to help the Apprenticeship Team understand your experiences while supporting your apprentice(s). It enables us to identify areas for improvement, celebrate apprentice successes, and enhance the support we provide.
If you manage or mentor more than one apprentice, please complete one feedback form per apprentice.

Anonymous, unless you choose to disclose anything that may identify you (e.g., the school/directorate you work in)
Stored confidentiality by West Sussex County Council, in line with relevant data protection regulations
By completing this survey, you consent to your responses being stored by West Sussex County Council’s Apprenticeships Team. Your feedback may be shared with relevant parties, including apprentices and training providers, as part of our ongoing quality monitoring and continuous improvement efforts.
